Sharing Health Data With a Doctor

The CONNEQT App makes it simple to share your health data with your doctor or healthcare provider. This feature is designed to enhance communication with your healthcare provider, empowering them to monitor your progress and make informed decisions about your care.

1. Open the CONNEQT App on your phone.

2. Tap the Care+ icon.

3. Select the report you want to share.

4. Once the report loads, tap the export icon.

5. Choose how you’d like to share it:

   • Email the PDF directly to your doctor.

   • Save the PDF and upload it to a patient portal.

   • Print the report and bring a copy to your appointment.
