Wide variation between repeated PWA tests on the same patient is most often caused by inconsistent test conditions, patient movement or talking, changes in posture or arm position, or small differences in cuff sizing or placement.
Step 1: Standardize Test Conditions
- Ensure the patient rests comfortably for at least 5 minutes before each test.
- Ask the patient to remain relaxed, silent, and still during measurements.
- Allow a brief 1–2 minute rest between repeated measurements during the same visit.
- When possible, perform follow-up measurements at the same time of day to account for diurnal blood pressure variation.
Step 2: Standardize Patient Position
- The patient should be seated with their back supported.
- The arm should be resting and supported at heart level.
Step 3: Verify Cuff Size
- Re-check cuff size using the index mark each time.
- Do not assume a previously used cuff size is still appropriate.
Step 4: Reproduce Cuff Placement Precisely
- Place the cuff on the bare upper arm.
- Center on the brachial artery, artery marker aligned, cuff as high as possible.
- Snug fit with two fingers of clearance; tubing not compressed.
Step 5: Use the Same Arm for Repeat Assessments
- For trend tracking, use the same arm for all repeat assessments.
Step 6: Document What Changed
If results vary, record any factors that may have affected readings, including:
- Recent exercise, stress, or medication timing.
- Whether the same arm and posture were used.
- Any talking or movement during capture.
