Care+ Benefits for Purchases Before October 10, 2025
Existing CONNEQT customers are receiving complimentary access as part of our rollout of Care+. Depending on your original purchase, your account may include complimentary Care+ access or On-Demand Assessments automatically added to your profile.
Your Benefits (applied automatically where possible)
- Device-only buyers → 10 On-Demand Assessments and 1 year of Care+ added
- Device + Arterial Assessment buyers (2- or 4-report) → Lifetime Care+ added
These benefits are provided at no additional cost and applied to your account where possible.
How to Check if You Have Care+*
- Open the CONNEQT Health app
- Tap the Hamburger Menu (☰)
- Go to Account Settings → Manage Plan
- If you see an active Care+ plan, you’re all set
If you still see a paywall, complete the quick Entitlement Intake form (link available in-app) and our team will update your account.
How to Check if You Have a 10-Pack of On-Demand Assessments
- Open the CONNEQT Health app
- Tap the Care+ tab
- Look just below the Begin On-Demand Assessment button, you’ll see the number of assessments remaining.
- If unused, you should see 10 remaining.
- If you’re still on your 30-day Care+ trial, the app will show unlimited. Once the trial ends, your 10 assessments will appear
If you still see a paywall, complete the quick Entitlement Intake form (link available in-app) and our team will update your account.
Why You Might Not See Your Benefits
Your purchase email and in-app email don’t match or your device serial number hasn’t been linked to your account
The Entitlement Intake form helps us resolve this quickly. Please have ready:
- Purchase email
- In-app account email
- Order ID
- Device serial number
- Platform (iOS or Android)
Once Activated
With Care+: Care+ includes unlimited On-Demand Assessments and a monthly Cardiology Report summarizing your arterial-health trends, central blood pressure, and key vascular insights over time.
With a 10-Pack: You can generate up to 10 On-Demand Assessments on your schedule and view results instantly in the app.
Your past readings and reports remain available even after the entitlements are used.
